Voluntary Self-Identification of Veteran Status (Pre-Offer)

This employer is a federal contractor subject to the Vietnam Era Veterans’ Readjustment Assistance Act of 1974 (VEVRAA), as amended by the Jobs for Veterans Act of 2002 (38 U.S.C. 4212). VEVRAA requires government contractors to take affirmative action to employ and advance in employment qualified:

  1. Disabled veterans
  2. Recently separated veterans
  3. Active duty wartime or campaign badge veterans
  4. Armed Forces service medal veterans

These classifications are defined as follows:

  1. Disabled Veteran – A veteran of the U.S. military, ground, naval, or air service who:
    • Is entitled to compensation (or who would be entitled to compensation but for receipt of military retired pay) under laws administered by the Secretary of Veterans Affairs; or
    • Was discharged or released from active duty because of a service-connected disability.
  2. Recently Separated Veteran – Any veteran during the three-year period beginning on the date of discharge or release from active duty in the U.S. military, ground, naval, or air service.
  3. Active Duty Wartime or Campaign Badge Veteran – A veteran who served on active duty during a war, or in a campaign or expedition for which a campaign badge has been authorized under the laws administered by the Department of Defense.
  4. Armed Forces Service Medal Veteran – A veteran who, while serving on active duty, participated in a U.S. military operation for which an Armed Forces service medal was awarded pursuant to Executive Order 12985.

Why We Ask for This Information

As a government contractor, we are required to collect this information to help measure the effectiveness of our outreach and recruitment efforts in support of VEVRAA. Providing this information is voluntary, and refusal to provide it will not subject you to any adverse treatment.

If you believe you belong to any of the categories of protected veterans listed above, please indicate by selecting the appropriate option.

Are you a veteran?

Your Privacy:

  • Any information you provide will be kept strictly confidential and used only in accordance with federal reporting and recordkeeping requirements.
  • Data will be stored securely and separately from your application or personnel file.
  • This information will not be used in making hiring or employment decisions.
  • You may update or withdraw your response at any time.

Additional Resources

Protected veterans may have additional rights under the Uniformed Services Employment and Reemployment Rights Act (USERRA). If you were absent from employment to perform service in the uniformed services, you may have reemployment rights to the position you would have attained had you not been absent due to service.

For more information, contact the U.S. Department of Labor’s Veterans’ Employment and Training Service (VETS) at 1-866-4-USA-DOL or visit www.dol.gov/vets.

Voluntary Self-Identification of Disability

(OMB Control Number 1250-0005; Current Version per U.S. Department of Labor Guidance)

Why We Ask for This Information

Because we do business with the U.S. government, we are required to reach out to, hire, and provide equal opportunity to qualified individuals with disabilities, in accordance with Section 503 of the Rehabilitation Act of 1973 (as amended).

To help measure our progress toward these goals, we ask applicants and employees to voluntarily self-identify whether they have a disability, or have ever had a disability. Completing this form is entirely voluntary, and your decision will not be used against you in any way.

If you choose to provide this information:

  • Your response will be kept confidential and stored separately from your application or personnel file.
  • The information will be used only in accordance with applicable federal regulations and for required reporting purposes.
  • You may update or withdraw your self-identification at any time.

If you already work for us, we may ask you to review or update this information periodically, as required by law.

How Do I Know If I Have a Disability?

Under federal law, a person is considered to have a disability if they have a physical or mental impairment or medical condition that substantially limits one or more major life activities, or if they have a history or record of such an impairment or condition.

Examples of disabilities include (but are not limited to):

  • Autism
  • Blindness or low vision
  • Cancer
  • Diabetes
  • Epilepsy
  • Hearing impairment
  • Major depression, bipolar disorder, PTSD, or other mental health conditions
  • Mobility impairments requiring the use of a wheelchair or other device
  • Intellectual disability
  • HIV/AIDS
Do you have a disability?

Your Privacy:

All information provided on this form is collected solely for compliance and reporting purposes. It will be:

  • Stored securely within our application and employment systems;
  • Accessed only by authorized personnel for compliance or audit reasons; and
  • Not shared or used in employment decision-making.

For more information about Section 503 or the obligations of federal contractors, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) at www.dol.gov/ofccp.

Reasonable Accommodation Notice: Federal law requires employers to provide reasonable accommodation to qualified individuals with disabilities. Please tell us if you require a reasonable accommodation to apply for a job or to perform your job. Examples of reasonable accommodation include making a change to the application process or work procedures, providing documents in an alternate format, using a sign language interpreter, or using specialized equipment.

PUBLIC BURDEN STATEMENT: According to the Paperwork Reduction Act of 1995 no persons are required to respond to a collection of information unless such collection displays a valid OMB control number. This survey should take about 5 minutes to complete.
